A calcium blood test measures the calcium circulating in your blood, not the calcium in your bones.

A calcium blood test measures the calcium circulating in your blood — not the calcium in your bones. Too much or too little can signal bone disease, thyroid or parathyroid disorders, or kidney disease, which is why an out-of-range calcium is followed up rather than corrected with a supplement.

The protocol

Unitsmg/dL
Standard rangelaboratory-specific (ranges vary by lab); total calcium is the routine measure and appears on both the BMP and the CMP
lowmay relate to parathyroid, kidney or vitamin D status, or to low magnesium
raisedmay point to parathyroid disorders, some cancers, or excess vitamin D or calcium supplementation
not_bone_densitya normal serum calcium says nothing about bone mineral density — that needs a DXA scan
total_vs_ionisedtotal calcium is the standard test; ionised (free) calcium is measured when abnormal albumin levels complicate the picture
albumin_dependencyroughly half of circulating calcium is bound to albumin, so a low albumin lowers total calcium without the free fraction changing — this is why albumin is read alongside
Neverdo not treat a low serum calcium with supplementation before the cause is identified
Contextread with albumin, and with vitamin D and PTH if the value is out of range
If raisedstop unsupervised high-dose calcium or vitamin D and ask a clinician; do not simply retest and hope
Bone healthweight-bearing and resistance exercise is the lifestyle lever for bone, not a higher serum calcium
Supplementslist every calcium and vitamin D supplement before the draw — both move the number

Full transcript

A calcium blood test measures the calcium circulating in your blood, not the calcium in your bones. A normal result says nothing about bone density — that needs a DXA scan. An out-of-range value is followed up, not corrected with a supplement. About half of it rides on albumin, so the two are read together.
